One in three people over the age of 85 have dementia, yet more than half have not been diagnosed.

The Faith Care Connection project seeks to lend critical support to vulnerable individuals in the community who are living alone and at risk for dementia. Our approach is unique in that we are identifying those at risk through church connections, with plans to expand to several other churches in the near future.

Working alongside pastoral care/senior ministers in churches, we have a licensed professional (social worker, nurse or physical therapist) reach out to those individuals identified to be at risk for dementia. Our role is to assess social isolation, depression, and quality of life; and provide recommendations, referral, and follow-ups with their care over the course of a year.
