SUPPORT FOR CONGREGANTS

One in three people over the age of 85 have dementia, yet more than half have not been diagnosed.

The Faith Care Connection project seeks to lend support to vulnerable individuals in the community who are living alone and at risk for cognitive changes or other changes due to aging. Our approach is unique in that we are identifying those at risk through church connections, with plans to expand to several other churches in the near future.

Working alongside pastoral care/senior ministers in churches, we have a licensed professional reach out to those individuals identified to be at risk for cognitive changes. Our role is to assess social isolation, depression, and quality of life; and provide recommendations, referrals, and follow-ups with their care over the course of a year.

No. The Faith Care Connection team is happy to provide referrals and resources for all seniors and caregivers as needed.

Care Consultation typically includes gathering information and providing appropriate referrals and resources from the community. Case Management refers to the facilitation of treatment plans to ensure appropriate medical care is provided to an individual.

Yes. We are happy to share our resources with all seniors and their caregivers as needed.

Our resources are primarily for individuals living within the greater Houston area. We do, however, have a few national resources.
